This can be done at the Canadian Embassy in Bangkok or the Canadian Consulate in Chiang Mai.  Note that you need certain documents for your affidavit as well as original documents if you are widowed or divorced.

The affidavit form needs to be completed and costs about $50 per affidavit. The process normally takes about 2 days as you need to return to collect the affidavit.
